YIZKOR BOOK OF REMEMBRANCE

The Adat Shalom Yizkor Book is a way for us to remember our family members and other significant people in our lives who have passed away.

This is the fourth year we are providing an opportunity to fill out the form in a web format. Please follow the instructions below.

INSTRUCTIONS:

The form contains 5 sections:

• Provide information about yourself or your family in Section I.
• If you would like to use your submission from last year, fill out Section II.
• If you would like to change, add to, or remove information from last year's entry, fill out Section III.
• Finalize your submission in Section IV
• Review the donation instructions in Section V.

This web entry form will continue to be available until September 9th. Entries received after September 9 cannot be included.

Two technical notes. (1) Make sure that "www." precedes "adatshalom.net" in your URL and (2) for anyone using Norton Internet Security (NIS) to protect their PC: this form does NOT play well with NIS. If you use NIS, to successfully submit an entry this is what you will need to do: 1) turn NIS off, 2) close your browser, 3) re-open your browser, 4) fill out the form and submit it, 5) turn NIS back on.

NOTE: The "Last Name" entered here is the name that will be used for sorting the Yizkor book entries, if more than one name is remembering someone and this is not the last name you want to have all the entries listed under, please state the name you want in the comments box below. If your family wants two separate listings at different places in the alphabet, complete two forms (one for each sort name).

Section II: Using Last Year's Entry Again:

Click Here to open the online version of last year’s Yizkor book which is being posted only for the sign-up period for this limited purpose. (If you cannot access the file it means that you do not have Adobe Acrobat, the application needed to read the Yizkor book. To download Adobe Acrobat, click PDF and then go back to the beginning of this Section and start over.)

Please review your entry from last year and verify that the information is EXACTLY as you want it to appear this year. The formatting in the online version of the Yizkor Book is not the way it will appear in print, so please ignore formatting and line breaks when reviewing your entry.

If last year’s information is correct, please do the following:

1.Make sure that your last name (bold black letters against turquoise background) listed in last year's entry exactly matches your last name entered in Section I above. If it does not, change your entry in Section I above to match last year’s.

2. Note the line number (bold black letters against yellow background) opposite your entry and type it in here:

If you do not want to use your last year’s entry go to Section III below.

Fill in In Memory of and Remembered by columns the way you want your listing to appear. Each box will hold up to 100 characters. Reminder: if you are changing your entry from last year, you need to retype it entirely from scratch.

Section IV: Finishing Submission

All your entries will be listed in the Yizkor Book together, alphabetized by your "Last Name" as listed above. If you would like your entry to look different, we will try to accommodate your request. Please let us know what you would like to do in the following box, which you can also use to give us any other comments:

CAUTION:After you hit the submit button below, you should see a confirmation message that contains a summary of your entry. You should also receive an e-mail containing the same information. If you do not see the confirmation message, hit the back button on your browser and resubmit or reenter the data (and add a comment in Section IV that it could be a duplicate submission). Section V: Payment

As always, donations should be mailed to the Congregation office along with your main High Holidays registration form.

Suggested donation: $18-36 per line multiplied by the number of lines = $_____

Please record your donation total on the main High Holiday registration form and return it with your registration form and payment.
